More than 20 years ago, a
US community Healthopolis was envisioned for 2020 based on a
Purchasing Population Health strategy that created appropriate
financial incentives for improved population health outcomes. The proposed
action steps to implement the strategy included:
1. Debate, accept, and
research a focus on value issues of outcomes and cost.
2. Create
outcome-based payment for integrated health delivery systems.
3.
Incorporate the nonmedical determinants and sectors. |
"If we could give every individual the right
amount of
nourishment and exercise, not too little and not too
much, we would have found the
safest way to health.
"
Hippocrates
"The American medical establishment is
disease-oriented as there is not a penny to be made in health." - Bob
Livingston
"If there is an extremely expensive,
minimally-beneficial treatment for a common problem, American medicine will
find it." - B. Mull
"The equivalent of 390
jumbo jets full of people are
dying each year due to likely preventable, in-hospital medical errors,
making this one of the leading killers in the US." - Dr. Samantha
Collier
"Americans
waste billions each year on drugs of dubious value.
Until we find a
way to fund quality controls on published research, the cost of our medical
care will continue to soar and our health will suffer." - Dr. John Abramson,
clinical instructor Harvard Medical School
"Medicine is now
a higher risk industry than
aviation. The chance of dying in an aviation accident is 1 in 2 million.
The risk of dying from a medical accident is 1 in 200!" - Dr. Lucian Leape, of
the Harvard Medical School
In 1984 a
complaint is filed by a group of US physicians with the UN Center for Human
Rights in Geneva, labeled
"A Complaint Against Medical Tyranny As Practiced in the USA: American
Medical Genocide".
"Each year in America there are:
12,000 deaths from unnecessary
surgeries;
7,000 deaths from medication errors in
hospitals;
20,000
deaths from other errors in hospitals;
80,000
deaths from infections acquired in
hospitals;
106,000
deaths from FDA approved correctly prescribed medicines. The total of
medically caused deaths in the US every year is 225,000." Dr. Barbara Starfield (Johns Hopkins School of Public
Health) July 26, 2000, Journal of the American Medical
Association, "Is US health really the best in the world?"
The
health care industry
fails to take into consideration the following critically important aspects of
a healthy human organism:
homeostasis;
insufficient
exercise; excessive caloric
intake; exposure to
tens of thousands of environmental toxins;
highly processed foods grown in
toxic chemically damaged soil;
adverse reactions of stress on the
immune system and life
processes.
American College of Surgeons estimates
only 5 to 30% of adverse reactions are
reported in surgical incident reports.
Standard medical pharmacology texts admits
relatively few doctors ever report adverse drug reactions to the FDA.
1901 Rockefeller Institute for Medical
Research opens. Initial
paradigm is based on the philosophy of Sanctorius.
1910 161 medical schools exist.
Flexner Report
is published on the basis of previous experiments and the paradigm of
allopathic medicine.
This highly influential report, sponsored by the
Rockefeller Foundation and the Carnegie Foundation, evaluated
medical schools and
restructure American medical education.
It sets up a new standard so
that allopathic medical schools can only be accredited if they show an emphasis
in fossil fuel byproduct based
chemical drug research and treatment.
Homeopathy and
nutritive approaches to medicine are
demonized.
Abraham Flexner, author of the report, is on the staff of
the Carnegie Foundation for the Advancement of Teaching.
1913 Rockefeller
Foundation establishes the International Health Division to
succeed the Rockefeller Sanitary Commission.
International Health Division of
the Rockefeller Foundation lays the foundations for methodologies of
health science research and the manner in which the development of medicine is
to be conducted.
'Health' institutions
including the World Health
Organization, National Science Foundation, National Institute
of Health and the National Cancer Institute are modeled on
International Health Division practices, policies, and research
processes.
The American Cancer Society is
formed.
1918 Eugenics becomes the top priority
of the Rockefeller
Foundation .
"The Foundation identifies public health education
as one of its principal areas of interest, and builds and endows the first
school of public
health at Johns Hopkins
University."
1919 In response to the
Flexner Report half the medical schools close - 81 allopathic medical
schools are now in operation.
August 5, 1937
National Cancer
Institute Act
Passage of the National Cancer Institute
Act enshrines cancer in the American psyche creating the National Cancer Institute.
The American Medical Association resists passage of the
National Cancer Institute Act as a threat to the role of organized
medicine in diagnosing and
managing a disease for which
they could see no "public managerial" function.
Health
Maintenance Organization Act of 1973
1979 to
1987 The surgery rate in the US
grows more than twice as fast as the population.
1995 "Over a million patients are injured in US hospitals each
year, and approximately 280,000 die annually as a result of these injuries.
Therefore, the iatrogenic death rate
dwarfs the annual automobile
accident mortality rate of 45,000 and accounts for more deaths than all
other accidents combined." - Journal of the American Medical
Association
1999 to 2007 Expenditures for
hospital care increases 90% from $462 billion to $873 billion.
2008 and 2009 Healthcare companies spend $635 million
lobbying. 166 former congressional aides involved in
shaping healthcare
legislation register to lobby for healthcare
companies.
"Healthy people aren't great
customers.
The money is in those in whom
an abnormality can be found.
The medical-industrial complex has made that relatively easy to do.
It develops diagnostic
technologies able to find smaller and smaller
abnormalities.
The medical-industrial complex has another way to find more
abnormalities: it simply narrows the definition of normal.
Take blood pressure.
In the past, relatively few
were said to have abnormal blood pressure.
Now a normal blood pressure is said to be
below 120/80.
This means that well over half the adult population
of the US is abnormal.
The same is true for
cholesterol. And
although it involves a smaller portion of the population, narrower definitions
of normal are expanding the number of people said to have
diabetes and
osteoporosis.
The new
abnormalities being found are generally not severe, but mild: they are not
life-threatening, and in many
patients they won't ever produce symptoms.
When the benefit of
treatment is known, it is known to be small - so small that many people,
sometimes hundreds, must be treated for one person to benefit.
What
is known is that others are invariably harmed in the process.
The
medical-industrial complex has systematically exaggerated the benefits and
minimized or ignored the harms.
All of us harbor abnormalities.
The construct drives the system to look for things to be wrong - a
search that will be successful in most of us.
We then feel more
vulnerable.
This induced vulnerability undermines the
very sense of well-being and
resilience that in many ways defines health itself." - H. Gilbert
Welch

Healthcare-associated
infections account for an estimated 1.7 million infections and 98,987
associated deaths in the year 2007.
Healthcare-associated infections in
US hospitals:
35,967 are for
pneumonia;
30,665 for bloodstream infections;
13,088 for urinary tract
infections;
8,205 for surgical site infections;
and 11,062 for
infections of other sites.
